Registration Form

Please fill out the following information.  Upon completion of your registration form you will be linked to a page to confirm that your registration form was recieved.  You will also recieve an email verifying that you are in the class as there is limited space available first come first serve basis.  On April 2, 2012 when you come in we will have this registration form.  Please bring you/your childs Alberta Health Care card.  If your child does not get in we will keep your registration form on file, if space becomes available we will notify you. Please register one person per form, if you have more than one child please put in the note section that you are registering a second child, when we recieve both forms we will take the time recieved of the first form for both.  If you have any questions or concerns in regards to your registration please contact Brandi Merritt @ 403-894-7085 or email @ merrittbrandi@hotmail.com.
